Amanita phalloides (death cap mushroom)

  • Pathophysiology
    • α-amanitin: blocks RNA polymerase → inhibition of mRNA transcription and protein synthesis → apoptosis
  • Clinical features
    • After 6–24 hours: gastrointestinal symptoms (diarrhea, vomiting, abdominal cramps) that resolve 24–36 hours after ingestion
    • After 2–4 days: renal and liver failure
    • Ingestion of a single cap may be lethal
